This is a home quarantine form that I used to ask for my hubby, hope it helps ex-pats who entered Vietnam at this hectic time wish to do quarantine at home.
Step by step how to ask for the home quarantine:
1. Print of the form in which you fill all information which I yellow in the file (don’t hesitate to ask me if you are not clear) + the copy of your covid injection certificate (3 days home quarantine for full vaccinated, 7 days quarantine for the person who has 1 injection) and 14 days quarantine for people who haven’t injected
2. Take both papers to the local health authorities. They will provide you with another paper, which you do not need to do anything.
In my case, I took both papers to the district health authority (Trung Tâm Y Tế Quận), they provided me with the 3rd one and they have it signed after asking me some questions. Then, I took it to the ward health authority to have it signed again. Finally, I sent it to the local committee to sign the final one.
=> Sum up, to complete that paper, you will need to have 3 signatures: the district health authority, the ward health authority, and the local committee
